Gnomish and goblin specialization
At high skill the engineer picks a branch:

You can switch specialization for a fee; some top schematics are branch-exclusive.
Raid and rare schematics
The strongest items (Force Reactive Disk etc.) drop in raids like Blackwing Lair. The repair bot and some scopes are rare world drops — check the auction house.
